This form must be completed by the parent/guardian of any student competitor who will be under 18 years of age on March 6, 2026.

Auto Dealers Association of Greater Philadelphia: Photo and Video Consent Form

I hereby grant Auto Dealers Association of Greater Philadelphia (ADAGP) and all of its entities, including Auto Dealers CARing for Kids Foundation and Philadelphia Auto Show, permission to use the name and likeness of the child named below in a photograph in any and all of its publications, promotional videos, social media posts, website entries, newsletters and any other program-related material, without payment or any other consideration. I understand and agree that these materials will become the property of ADAGP and all of its entities and will not be returned. I hereby irrevocably authorize ADAGP and all of its entities to edit, alter, copy, exhibit, publish or distribute such photo for purposes of publicizing its programs or for any other lawful purpose. In addition, I waive the right to inspect or approve the finished product, including written or electronic copy, wherein the likeness appears. Additionally, I waive any right to royalties or other compensation arising or related to the use of the photograph. I hereby hold harmless and release and forever discharge ADAGP and all of its entities from all claims, demands, and causes of action which I, my heirs, representatives, executors, administrators, or any other persons acting on my behalf or on behalf of my estate have or may have by reason of this authorization.
